This page may be filled out by the parents/guardians of the scout.  Use this form to provide the financial aid committee with any information you think may help them in making a decision.  You do not have to fill out all parts of the form.  You may provide as much, or as little information as you wish.  You are not required to submit this form.  If you do not submit this form, decisions will be made based on part one only.  Information on this form will be available only to members of the Financial Aid subcommittee and will be held in strictest confidence.
